This Service is a client centred Service for people when their health and wellbeing is significantly affected by mental ill health. The Home Support Service provides practical, emotional and social support for people within their own home, homeless accommodation or community setting in Stirling District.
The support is provided in a flexible, client-centred way, appropriate to individual lifestyle, culture, age and need. The Service is delivered via home visits and one to one support, but there is the option of facilitating support meetings in the local community and initial supported attendance to specific groups.
What will the Home Support Service provide?
- Support for clients to retain their tenancies
- Practical support in budgeting, food preparation and home maintenance
- Assistance in developing decision making skills
- Information
- Basic advocacy
- Liaison with other agencies
- General counselling
- Support in social skills and development of community involvement
- Support in order to make informed choices in daily living
How to be referred
The Service has a referral system. Home Support encourages the person requesting the Service and the referring agency to jointly complete the Referral Form.
Referrals are taken from – Social Workers, Psychiatrists, GPs, CPNs, Key Workers or other voluntary sector agencies.
Criteria for referral
Adults 18 + years, living within Stirling Council areas, experiencing or recovering from mental ill health.
An assessment would then be undertaken to determine if the potential client was eligible for the Service under Supporting People criteria.
